Hip involvement and grade 4 sacroiliitis in ankylosing spondylitis

Destructive disease in both right and left hips in a patient with sacroiliitis. The heads of both femurs show deformities and irregularities on the surfaces, more in the left than in the right. Subchondral cysts are observed in both. Irregular bone formation indicating enthesopathy is noted at the ischial tuberosities. In addition, both sacroiliac joints show grade 4 sacroiliitis.
