Remodeling after treatment for developmental dysplasia of the hip
This female patient was diagnosed with dislocated left hip shortly after birth. The patient failed Pavlik harness treatment as well as an attempted closed reduction.
(A) Antero-posterior (AP) radiograph of pelvis at age 10 months, just before open reduction. Note lack of acetabular development and failure of ossific nucleus to form.
(B) AP of pelvis 3 months after open reduction (age 13 months). Note early acetabular formation.
(C) Frog lateral of pelvis 3 months after open reduction. Note appearance of ossific nucleus (arrow) and early acetabular formation.
(D) AP of pelvis at age 1 year, 6 months. Ossific nucleus remains small.
(E) AP radiograph of pelvis at age 2 years, 6 months. Ossific nucleus catching up to opposite side and acetabulum continues to normalize.
(F) AP radiograph of pelvis at age 4 years, 4 months. The femoral heads are equal in size and shape. The left acetabulum remains slightly dysplastic.
(G) AP radiograph of pelvis at age 7 years (6 years and 2 months after open reduction). Except for postsurgical changes along the ilium, the hips appear similar. The patient has full pain-free motion and function, and will continue to be monitored until skeletal maturity.
