(A) The "black heel" results from the formation of tiny petechiae in the epidermis of the heel caused, in most cases, by friction with tight, ill-fitted sport shoes. On dermoscopy, reddish to black, globular pigmentation on the ridges (the pebbles on the ridges) is characteristic.
