In this patient with biliary obstruction, attempts
to cannulate the bile and pancreatic ducts were unsuccessful using standard
guidewire-assisted cannulation. Thus, needle-knife papillotomy was performed by
starting the incision from the ampullary orifice (A) and extending it
upwards until the bile duct was visualized (B) and cannulated (C).
