Intensity and degree of debilitation

Relationship to menstrual cycle

Relationship to eating, bowel, or bladder function

Relationship to sexual or physical activity

Associated symptoms, such as nausea or diarrhea

Use of analgesics or hormonal therapy and their effectiveness

Urologic

History of recurrent urinary tract infections

Relationship to sexual activity or diaphragm use

Use of suppressive antibiotics

Dysuria, urgency, or urinary frequency

Difficulty voiding

Urinary incontinence or leakage

Hematuria

Pelvic relaxation

Sense of vaginal pressure or that something is "falling out"

Difficulty with penetration during intercourse

Difficulty with voiding or defecating

Need to "splint," or apply vaginal pressure with above activities

Incontinence of urine, stool, flatus
