Diagnostic criteria for functional abdominal pain disorders in children and adolescents
Universal criterion: The symptoms cannot be completely explained by another medical condition after appropriate evaluation
Functional dyspepsia
In addition to the universal criterion, ≥1 of the following bothersome symptoms on ≥4 days/month for ≥2 months:
  • Persistent sensation of fullness after eating
  • Feeling full after eating small amounts of food
  • Epigastric burning or pain unrelated to bowel movements

Subtypes:

  • Postprandial distress syndrome: Early or persistent sensation of fullness prevents finishing a typical meal
  • Epigastric pain syndrome: Bothersome epigastric pain or burning not relieved with having a bowel movement or passing gas
Irritable bowel syndrome
In addition to the universal criterion, abdominal pain on ≥4 days/month for ≥2 months that is associated with ≥1 of the following:
  • Pain during bowel movements
  • Altered stool appearance
  • Altered stool frequency - either more or less frequent
    • For children or adolescents with less frequent stools, abdominal pain persists after resolution of constipation (pain that resolves with resolution of constipation indicated functional constipation, described below)
Abdominal migraine

Abdominal pain is the most severe and troublesome symptom.

In addition to the universal criterion, all of the following:
  • Recurrent sudden episodes of intense abdominal pain (diffuse, midline, or periumbilical) lasting ≥1 hour on ≥2 occasions within 6 months
  • Weeks or months between episodes
  • The pain interferes with normal activities
  • The symptoms and pattern vary from patient to patient
  • The pain is associated with ≥2 of the following:
    • Anorexia
    • Nausea
    • Vomiting
    • Headache
    • Photophobia
    • Pallor
Functional abdominal pain - not otherwise specified (NOS)
In addition to the universal criterion, all of the following:
  • Occurs ≥4 times/month for ≥2 months
  • Whether constant or intermittent, the abdominal pain is not exclusively related to physiologic events (eg, menstruation, eating)
  • Does not meet criteria for functional dyspepsia, irritable bowel syndrome, or abdominal migraine
Functional constipation*
In addition to the universal criterion, ≥2 of the following on ≥1 day/week for >1 month and does not meet criteria for irritable bowel syndrome:
  • For children with a developmental age of ≤4 years: ≤2 bowel movements in the toilet per week
  • ≥1 episode of fecal incontinence per week
  • Painful or hard bowel movements
  • Large diameter stools
  • Purposeful stool retention or retentive posturing
  • A large fecal mass in the rectum
* Functional constipation is not categorized as a functional abdominal pain disorder in the Rome IV classification. However, it is included in this table as a common cause of chronic abdominal pain in children.
