In this transthoracic echocardiogram, the continuous wave Doppler recording obtained from the pulmonary vein (PV), shows systolic flow reversal or retrograde systolic flow, which is the hallmark of severe mitral regurgitation (MR) and is the flow analogue of the v-wave seen on the pulmonary artery catheter wedge pressure tracing.
