Allograft rejection after immunosuppression withdrawal

Abdominal and pelvic CT scan shows ill-defined margins and heterogenous enhancement of kidney allograft (arrow), findings that represent rejection. The surrounding mesenteric fat and abdominal wall musculature have inflammatory changes. This patient, in whom immunosuppression had been completely withdrawn, eventually required a transplant nephrectomy.
