Despite warnings of potential for cross-reaction contained in the manufacturers' labeling of the nonantimicrobial sulfonamides listed in this table, there is minimal evidence for cross-reactivity for these agents in patients who are allergic to sulfonamide antimicrobials.
Sulfasalazine is an exception and can cause a cross-reaction in patients with allergies to sulfonamide antimicrobials because it releases sulfapyridine (a cross-reactive arylamine sulfonamide) in contact with gut flora. Thus, use of sulfasalazine is contraindicated in patients with a significant allergy to sulfonamide antimicrobials.
