Paraesophageal hernia sac dissection along the left crus

Sac dissection begins along left crus at approximately the 3 o'clock position and extends anteriorly toward 12 o'clock. Peritoneum is left on the crus and the dissection follows the plane between the muscle fibers and the hernia sac.
