This algorithm summarizes an approach to interpreting EBV-specific antibody tests in patients with suspected infectious mononucleosis due to acute EBV infection. Because EBV-specific antibody tests change over time, results must be interpreted in the context of symptoms (refer to inset). Please refer to related UpToDate content for additional details.

CMV: cytomegalovirus; EBNA: Epstein-Barr virus nuclear antigen; EBV: Epstein-Barr virus; HHV: human herpes virus; HIV: human immunodeficiency virus; IgG: immunoglobulin G; IgM: immunoglobulin M; VCA: viral capsid antigen.

* Send additional studies to evaluate for non-EBV illness, including CMV, HIV, toxoplasma, or HHV-6 and HHV-7.

¶ Symptoms may be related to EBV or non-EBV illness. Correlate test results with the clinical scenario and time course of symptoms. In patients with prolonged symptoms, EBV is less likely.

Δ EBNA may not develop in up to 5% of previously healthy patients and 20% of immunosuppressed patients.
