* Indications for irradiation are listed separately in UpToDate. Communication regarding the need for irradiation differs by institution and may include an order in the medical record or discussion between the primary team and the transfusion service. Some institutions use universal irradiation or irradiation based on where the patient is being treated (eg, hematology/oncology service). Clinicians should familiarize themselves with their institution's policies and those of surrounding institutions.

¶ Monoclonal antibodies that interfere with pretransfusion testing include anti-CD38 (daratumumab, isatuximab) and anti-CD47 (investigational).

Δ Many institutions have an MSBOS, which lists procedures that require a type and screen or type and crossmatch, along with the number of blood components that should be reserved for specific procedures.
