Oral antifungal regimens for tinea pedis

Itraconazole	3 to 5 mg/kg per day in single or divided doses (maximum: 200 mg twice daily)	1 week
Fluconazole	6 mg/kg once weekly (maximum: 150 mg once weekly)	2 to 6 weeks
Griseofulvin^*	Microsize formulations: 10 to 20 mg/kg once daily (maximum: 1000 mg per day) Ultramicrosize formulation: 5 to 15 mg/kg once daily (maximum: 750 mg per day)	4 to 8 weeks

Oral therapy is reserved for tinea pedis refractory to topical antifungal therapy.

* Griseofulvin is available as microsize (ie, oral suspension and 500 mg tablet) and ultramicrosize (ie, 125 and 250 mg tablets) formulations; ultramicrosize griseofulvin has greater bioavailability and requires lower total daily doses. Administration with a high-fat meal increases absorption of both formulations and is required for microsize griseofulvin.
