Dosing: Adult
Pain relief

Pain relief: Topical:

Cream: Apply to the affected area 3 to 4 times daily.

Patch: Apply 1 patch to affected area not more than 3 to 4 times daily; leave in place for no more than 8 hours.

Contraindications

OTC labeling: When used for self-medication, do not use if allergic to aspirin, camphor, menthol, methyl salicylate, or any component of the formulation.

Warnings/Precautions

Concerns related to adverse effects:

• Burns: May cause serious burns (eg, first- to third-degree chemical burns) at the application site. In some cases, hospitalization has been required. Discontinue use and seek medical attention if signs of skin injury (eg, pain, swelling, blistering) occur following application (FDA Drug Safety Communication 2012).

Special populations:

• Pediatric: The American Academy of Pediatrics suggests that camphor be avoided in pediatric patients because of its lack of efficacy and the well-documented toxicity that may occur with use (AAP 1994). Although toxicity most often results from oral ingestion of topical or inhalation products, combined dermal and inhalation exposure has resulted in toxicity (Love 2004).

Dosage form specific issues:

• Cream: Do not use tight bandages on the treated area.

• Topical patch: Self-medication (OTC use): For external use only and should not be applied to wounds or damaged skin; do not use concurrently with other topical analgesics. Do not use tight bandages on the treated area. Do not use with a heating pad.

Other warnings/precautions:

• Self-medication (OTC use): When used for self-medication, contact health care provider if allergic to aspirin or salicylates prior to use. Discontinue use and contact health care provider if symptoms persist >7 days, condition worsens or recurs, or if rash, irritation, or itching develops. Do not use on open wounds, damaged skin, or rashes; in the eyes or on any mucous membranes.

Administration: Adult

Topical: Apply topically; for external use only. Do not apply to eyes, wounds, rashes, damaged skin, or mucous membranes. Do not bandage treated area tightly.

Topical patch: Apply patch externally to clean and dry affected area. Backing film should be removed prior to application. Do not use a heating pad after application or expose treated area to direct sunlight. Do not use concurrently with other topical analgesics.

Use: Labeled Indications

Pain relief: Temporary relief of minor aches and pains of muscles and joints associated with arthritis, bruises, simple backache, sprains, and strains.

KIDs List: Camphor, when used in pediatric patients <18 years of age, is identified on the Key Potentially Inappropriate Drugs in Pediatrics (KIDs) list and should be used with caution due to risk of seizures (weak recommendation; low quality of evidence) (PPA [Meyers 2020]).

KIDs List: Salicylates, when used in pediatric patients <18 years of age with suspicion of viral illness (influenza, chickenpox), are identified on the Key Potentially Inappropriate Drugs in Pediatrics (KIDs) list and should be used with caution due to risk of Reye syndrome (weak recommendation; very low quality of evidence) (PPA [Meyers 2020]).

Mechanism of Action

Camphor: Rubefacient and mild analgesic.

Menthol: Dilates the blood vessels, causing a sensation of coldness followed by an analgesic effect; relieves itching.

Methyl salicylates: Decreases inflammation and an analgesic.
