Dosage guidance:
Clinical considerations: For use as chronic immunosuppressive therapy in patients with anti-acetylcholine receptor (AChR) antibody–positive or anti-muscle-specific tyrosine kinase (MuSK) antibody positive myasthenia gravis as monotherapy (eg, in patients who cannot tolerate glucocorticoids), as bridge therapy with slower-acting agents, or in combination with glucocorticoids in patients with glucocorticoid-resistant or glucocorticoid-dependent disease. Delay administration in patients with active infection until the infection is resolved. Immunization with live or live-attenuated vaccines is not recommended during treatment due to transient reduction in IgG levels during therapy (Ref).
Myasthenia gravis, generalized (alternative agent):
SUBQ: Dose according to body weight (see table below) once weekly for 6 weeks. Subsequent treatment cycles (dose according to body weight once weekly for 6 weeks) may be administered based on clinical evaluation and no sooner than 63 days from the start of the previous treatment cycle.

Concerns related to adverse effects:
• Aseptic meningitis: Aseptic meningitis has been reported in patients treated with rozanolixizumab. If symptoms of aseptic meningitis occur, initiate appropriate diagnostic workup and treatment.
• Hypersensitivity reactions: Hypersensitivity reactions, including angioedema and rash, may occur, typically within 1 day to 2 weeks of administration. Monitor during and after administration. If a hypersensitivity reaction occurs, initiate appropriate treatment.
• Infections: Rozanolixizumab may increase risk of infection. Delay administration in patients with active infection until the infection is resolved.
Dosage form specific issues:
• Polysorbate 80: Some dosage forms may contain polysorbate 80 (also known as Tweens). Hypersensitivity reactions, usually a delayed reaction, have been reported following exposure to pharmaceutical products containing polysorbate 80 in certain individuals (Isaksson 2002; Lucente 2000; Shelley 1995). Thrombocytopenia, ascites, pulmonary deterioration, and renal and hepatic failure have been reported in premature neonates after receiving parenteral products containing polysorbate 80 (Alade 1986; CDC 1984). • Immunizations: Evaluate need to administer age-appropriate vaccines prior to initiation of new treatment cycle. Avoid the use of live or live-attenuated vaccines in patients undergoing rozanolixizumab treatment.

SUBQ: Administer only via SUBQ infusion using an infusion pump. Administer using an infusion set with a 26 gauge or larger needle and tubing ≤61 cm in length. According to the manufacturer, it is recommended to use a pump capable of administering a preset volume (vials contain excess volume for priming of the infusion line), with syringe pump occlusion alarm limits at the maximum setting. After infusion set is attached to syringe, prepare pump and prime tubing according to device manufacturer's instructions; administer immediately after priming infusion set. Choose an administration site on lower right or left part of abdomen below the navel; do not infuse where skin is bruised, hard, red, or tender; avoid areas with scars, stretch marks, or tattoos; rotate sites for subsequent administrations. Clean administration site with alcohol wipe, insert infusion set needle into site and secure to skin with sterile gauze and tape or transparent dressing. Infuse at a constant flow rate up to 20 mL/hour. Following completion of infusion, do not flush infusion line, as infusion volume takes into account volume losses in line. Discard remaining solution.
Monitor for signs/symptoms of hypersensitivity during administration and for 15 minutes after infusion is complete.

Management: Give non-live/inactivated/non-replicating vaccines at least 2 weeks prior to starting immunosuppressants when possible. Rozanolixizumab is a humanized monoclonal antibody (IgG4). Human IgG crosses the placenta. Fetal exposure is dependent upon the IgG subclass, maternal serum concentrations, placental integrity, newborn birth weight, and GA, generally increasing as pregnancy progresses. The lowest exposure would be expected during the period of organogenesis and the highest during the third trimester (Clements 2020; Palmeira 2012; Pentsuk 2009).
Based on data from animal reproduction studies conducted in monkeys using doses 10 and 30 times the maximum recommended human dose, in utero exposure to rozanolixizumab may cause fetal harm.
When treatment for myasthenia gravis in pregnancy is required, agents other than rozanolixizumab are currently recommended (Sanders 2016).
It is not known if rozanolixizumab is present in breast milk.
Rozanolixizumab is a humanized monoclonal antibody (IgG4). Human IgG is present in breast milk; concentrations are dependent upon IgG subclass and postpartum age (Anderson 2021).
According to the manufacturer, the decision to breastfeed during therapy should consider the risk of infant exposure, the benefits of breastfeeding to the infant, and the benefits of treatment to the mother.
