Approach to an abnormal mean platelet volume (MPV)
Clinical judgement is required to interpret an abnormal MPV, especially when the value is only slightly abnormal. Review of a freshly prepared blood film by an experienced evaluator is often helpful to confirm an abnormal finding and evaluate other hematologic abnormalities. While these listings may be helpful in suggesting possible explanations, nearly all of the possible conditions have more specific tests to assist with accurate diagnosis.

MPV: mean platelet volume; ITP: immune thrombocytopenia; TPO-RA: thrombopoietin receptor agonist; MDS: myelodysplastic syndrome.

* Some artifactual results for the MPV may indicate a serious condition, such as a thrombotic microangiopathy with schistocytes and thrombocytopenia. Some artifacts may be analyzer-specific. Hematologist consultation may be appropriate in acutely ill patients with thrombocytopenia and other abnormal results on the CBC.

¶ An abnormal MPV may be seen in certain hematologic conditions with abnormalities of red blood cells (RBCs) or white blood cells (WBCs). In these cases, the evaluation should focus on the abnormal RBC and/or WBC findings.

Δ Examples include recovery of hematopoiesis upon correction of vitamin B12 or folate deficiency, or recovery from a bone marrow insult such as alcohol, infection, or toxin.
