Football sign in necrotizing enterocolitis (NEC)

The football sign is a rare radiographic finding in neonates with NEC, indicating massive pneumoperitoneum. Free air outlines the entire abdominal cavity (arrows), creating a large oval radiolucency in the shape of an American football.
