Two options for positive pressure ventilation in the patient with a t-tube in place

Positive pressure ventilation in a patient with a t-tube is challenging because the tracheal limb is open at the cranial end.

Panel A shows ventilation from above using a supraglottic airway, with the extratracheal limb capped.

Panel B shows ventilation using an adaptor connecting the breathing circuit to the extratracheal limb, with an SGA clamped to prevent gas from escaping proximally.
