Implications of a positive APOL1 genetic test result

Implications of a positive APOL1 genetic test result
G1 and G2 are the two common APOL1 risk alleles that contribute to kidney disease risk; G0 refers to a low-risk haplotype that does not contribute to risk. Individuals with biallelic risk haplotypes (G1/G1, G1/G2, and G2/G2) are at risk for certain forms of kidney disease and associated complications. Individuals who are heterozygous (G0/G1 or G0/G2) are not at increased risk but can pass the risk allele to their children. Refer to UpToDate for important information about these risk alleles and about disclosing APOL1 genetic test results.

BP: blood pressure; CLIA: Clinical Laboratory Improvement Amendments.

* Confirm that testing was done in a CLIA-certified or other nationally certified laboratory, and that the individual has received pretest counseling and wants to receive their results. Determine whether they are positive for biallelic risk alleles (homozygous or compound heterozygous for the G1 and/or G2 allele) or are heterozygous.

¶ Kidney function monitoring includes:
  • Serum creatinine
  • Urinalysis
  • Urine albumin-to-creatinine ratio

Δ Kidney disease risk factors other than APOL1 may be present.
