Ruptured intracranial dermoid cyst
37-year-old with a ruptured intracranial dermoid cyst. Noncontrast CT (A, B) demonstrates an oblong, extra-axial mass in the right prepontine cistern and paracavernous sinus space with areas of marked hypoattenuation (arrowheads). On T2-weighted (C), FLAIR (D), and T1-weighted (E) MRIs, there are areas of both high and low signal intensity within the mass that are isointense with fat due to the sebaceous contents of the dermoid cyst. On a magnified T1-weighted (F) image through the basilar cisterns, note small foci of hyperintensity (arrows), consistent with droplets of sebum and indicating cyst rupture. Incidental note is made of sebaceous cyst in the right occipital scalp.
