This algorithm describes the general approach to selecting a suitable mode of NIV for patients with chronic respiratory failure due to chest wall and neuromuscular diseases. Refer to UpToDate content on preparation steps and titration modules for NIV trials.

NIV: noninvasive ventilation; BPAP: bilevel positive airway pressure; VAPS: volume-assured pressure support.

* Pressure-limited devices are the preferred device for reasons listed in the inset table.

¶ Volume-limited ventilators are generally less commonly used than in the past and their use has largely been supplanted by pressure-limited devices.
