Areolar eczema is a common problem during breastfeeding, characterized by itching and burning of the areola and nipples, with a red scaly rash. During an acute presentation, vesicles, crusting, and erosions are seen in the affected areas (panel A), whereas in the chronic state, the areas are generally dry, erythematous, lichenified, and scaling (panel B). Areolar eczema is more common in people with a history of eczema in other areas of the skin. Contributing factors may include irritants (soaps or fragrances) or allergic reactions to breast pads or topical medications such as lanolin, antifungals, or antibiotics.
