Seventeen-year-old with SLE and kidney transplant presents with seizures on tacrolimus. FLAIR images show classic subcortical white matter edema consistent with RPLS, predominantly involving occipital, parietal, posterior temporal, and cerebellar white matter, but also in right frontal white matter and both caudate nuclei.
