* Doses based on actual (total) body weight may be rounded to nearest 10 mg depending on local practice and administered once every 24 hours. Further studies are needed to evaluate the appropriateness of daptomycin dosing by total body weight in children and adolescents who are obese (ie, body mass index >30 mg/m2); closer monitoring for toxicity may be warranted in such cases.
