Brain magnetic resonance imaging (MRI) of a 26-year-old with left mesial temporal lobe sclerosis

Brain magnetic resonance imaging (MRI) of a 26-year-old with left mesial temporal lobe sclerosis
26-year-old with temporal lobe epilepsy and left mesial temporal sclerosis (MTS). Coronal T2-weighted MRI images (A and B) demonstrate decreased size and increased signal intensity of the abnormal left hippocampus (oval) and asymmetric atrophy of left fornix (arrow). Coronal FLAIR (C) shows asymmetric hyperintensity of small left hippocampus (oval). Magnified image (D) of temporal lobes shows abnormal morphology of small left hippocampus, with absence of normal stratum lacunosum moleculare (arrowheads in right hippocampus), asymmetric enlargement of left temporal horn (arrow), and global reduction in volume of white matter in left temporal lobe.
FLAIR: fluid-attenuated inversion recovery.
