Once the tracheal tube introducer (ie, bougie) is in place within the trachea, a 6.0-size tracheal tube is railroaded over the bougie until the vocal cord line above the cuff is advanced several centimeters into the stoma. Avoid placing the tracheal tube too deeply into a mainstem bronchus; a 6- to 8-cm insertion depth is appropriate for most adults.
