CT: computed tomography; PAX: paired box protein; FOXO1: forkhead box protein O1.
* Unfavorable histology: alveolar histology or PAX-FOXO1 fusion.
¶ Full metastatic workup includes chest CT, bone scan, and bilateral bone marrow examination.
