Common reasons hospice patients present to the emergency department
Patient and caregiver factors
  1. Inadequate symptom control (eg, pain; dyspnea).
  1. Malfunction or loss of support devices (eg, tracheostomy or gastrostomy tube).
  1. Stress, fear, or otherwise inability to cope with impending loss of life.
  1. Conflict about life-prolonging treatments. These may be treatments that were discontinued (eg, chemotherapy) or interventions that were never used (eg, hemodialysis).
  1. Patients and caregiver(s) have differing philosophies of care.
  1. Caregiver fatigue or inability to cope with patient needs.
  1. Automatic ingrained response to perceived patient distress.
Hospice-related factors
  1. Failure or inability to address patient needs in a timely manner.
  1. Poor communication with the patient or family.
  1. Equipment failure without timely repair or replacement (eg, home oxygen or nebulizer).
  1. Call initiated by hospice (eg, unable to provide a specific type of care).
