The rectosigmoid junction is grasped with an atraumatic bowel grasper and delivered out of the pelvis, placing the rectosigmoid mesocolon on tension. This allows the right lateral superior pelvic peritoneal reflection (gutter) to be incised to initiate a medial-to-lateral dissection.
